Insight 20 — When We Are Not Sure of the Root Canal Treatment, Designing the Core and Post Is a Treatment Decision

In this case, the tooth had been crowned about two years earlier. At the time of treatment, the quality of the root canal treatment could not be fully assessed, and it had been performed by a clinician with an unknown track record. The endodontist had given a confirmation letter.
For this reason, in designing the coronal reconstruction, the goal was not merely to create maximum strength, but preserving the possibility of future re-intervention was taken as a principle.
Today the patient presented with pain and signs of infection, and on specialist examination it became clear that one of the canals had been missed in the initial treatment.

In cases where there is no complete certainty about the quality of the root canal treatment, the choice of post type and the design of the core must be such that:
re-access to the canals is preserved
if retreatment is needed, no unnecessary destruction of the root structure is created
the treatment path is not «locked» in the future
today's decision does not limit tomorrow's freedom of decision
This approach is independent of the type of core material and relates to the logic of the design, not the name of the material.
In such conditions, the coronal reconstruction is not just a restorative step;
it is part of the risk-management strategy in root canal treatment.
Sometimes the right choice is the one that is reversible.
